Florida Blue Your Health Care Network Your Health Care Network The MCASF Local 725 Health & Welfare Trust is self-funded, which simply means we pay for each approved claim submitted by your provider for you as an eligible participant. The MCASF Local 725 Health & Welfare Fund selected Florida Blue to be our healthcare network and to adjudicate (pay) the approved claims. Florida Blue works with the Board of Trustees to ensure all eligible participants receive the best coverage at reasonable cost to the Fund. As an Apprentice 2nd, 3rd, 4th or 5th year, you receive a Summary of Benefits & Coverage (SBC) each year in January. This governmental required document provides a summary of coverage and any applicable co-payments or co-insurance. Click on the button below to read the latest SBC.
